 NCCSIA All-State Nomination Forms
Attached are nomination forms for the 2004 North Carolina Collegiate Sports Information Association (NCCSIA) All-State Softball and Baseball Teams (College and University Divisions).

While instructions are included on the nomination form, please note the following highlights:
* Any sports information director or sports information representative can nominate a student-athlete for the team. The student-athlete should be a starter or key reserve who has played in at least 50 percent of the team's games.
* Nominations should be typed.
* The college teams are comprised of softball and baseball student-athletes from NCAA Division II, NCAA Division III and NAIA institutions.
* The university teams are comprised of softball and baseball student-athletes from NCAA Division I institutions.
* The teams will include two pitchers, four infielders, three outfielders and a designated player. While anyone can nominate, the final all-state selections will be made by dues-paying members of the NCCSIA.
